CASE MANAGER JOB DESCRIPTION

 

REPORT TO: Program Manager

 

JOB SUMMARY:

Under direct supervision of the Program Manager, counsels and aids ex-offenders and families requiring one or more forms of psychosocial services; interviews clients experiencing problems with personal and family adjustments, finances, employment, food, housing, physical and mental impairments, and/or similar areas; secures and evaluates applicable related information; refers clients to community resources and other organizations.

 

Applicant will be responsible for developing, assessing, facilitating, and monitoring all components of assigned caseloads. All duties will be performed based on administrative training and direct supervision from the Project Manager. The Case Manager will maintain a flexible, organized and efficient work schedule and is subject to work overtime and be on-call.

 

Applicant must possess excellent organizational, writing, analytical and processing skills. He/she should be able to perform conflict resolution or crisis intervention and have excellent communication skills. The Case Manager will conduct individual and/or group counseling, seek advocacy for clients and embrace cultural diversity awareness. The Case Manager must have the ability to work as a direct care staff with a diverse population

 

DUTIES AND RESPONSIBILITIES:

MINIMUM JOB REQUIREMENTS:

Bachelor’s degree in a social service or related field, Masters Degree Preferred with Licensure from the State of Texas, with (1) year experience working with at risk/ high risk populations; may include part-time, volunteer or internship experience.
